Registered Nurse – Hybrid Assessor | Long Island

We’re currently seeking experienced Registered Nurses in Long Island, NY for a long-term hybrid assessor program. 

As a Hybrid Nurse Assessor, you’ll conduct comprehensive health assessments for patients enrolled in managed long-term care and community programs. Your focus will be on quality, accuracy, and compassion—not speed or volume. You’ll use your clinical expertise to evaluate patients’ functional abilities and care needs, both in-person and remotely.

Key Responsibilities

  • Complete  2 in-person assessments per field day

  • Conduct 3 telehealth assessments on remote workdays, with dedicated prep and buffer time built in.

  • Perform thorough evaluations of patients’ ADL/IADL abilities, cognitive function, and overall health status.

  • Accurately document findings using electronic systems and standardized tools.

  • Collaborate with interdisciplinary care teams to ensure care plans are accurate and up to date.

  • Maintain compliance with all state and program standards.

Requirements

Essential:

  • Active Registered Nurse (RN) license in New York State.

  • Minimum 2 years of home health experience (CHHA, LHCSA, MLTC, or equivalent).

  • Strong patient assessment skills and clinical judgment.

  • Proficiency with electronic documentation systems and remote communication technology.

  • Ability to travel within assigned region

Desirable:

  • Previous experience conducting health or needs-based assessments.
