Registered UAS Nurse
Are you an experienced RN in Westchester County looking to expand your skills in assessment-based care?
We are hiring Registered Nurse Assessors to conduct Uniform Assessment System (UAS) evaluations for the New York Independent Assessor Program (NYIAP). This is a great opportunity for nurses interested in transitioning into an assessment-focused role while maintaining clinical expertise.
As a Registered Nurse Assessor, you will receive comprehensive paid training to perform health assessments for Medicaid recipients (ages 19+), determining their eligibility for long-term community-based services. Assessments are conducted via telehealth and in-home visits, offering a flexible, consumer-driven schedule.

Key Responsibilities:
- Complete telehealth and in-home UAS assessments after receiving training
- Review and summarize medical records to determine care needs and eligibility
- Document assessments accurately in the UAS-NY online system
- Collaborate with the clinical team to ensure precise evaluations
- Stay up to date on state guidelines and regulations
- Travel within your assigned region for in-home visits
Requirements:
- Active RN license in New York State
- Minimum two years of one-on-one patient care experience in a home setting
- Strong organizational and communication skills
- Basic proficiency in Microsoft Word, Outlook, and Zoom
- Valid New York State ID and ability to travel for in-home visits
- Reliable high-speed internet for telehealth assessments
