We are hiring Registered Nurse Assessors to conduct assessment evaluations in New York. This is a great opportunity for nurses interested in transitioning into an assessment-focused role while maintaining clinical expertise.

As a Registered Nurse Assessor, you will receive comprehensive paid training to perform health assessments, determining their eligibility for long-term community-based services. Assessments are conducted via telehealth and face-to-face, offering a flexible, consumer-driven schedule.

Key Responsibilities:

Complete telehealth and face-to-face assessments after receiving training
Review and summarize medical records to determine care needs and eligibility
Document assessments accurately in online system
Collaborate with the clinical team to ensure precise evaluations
Stay up to date on state guidelines and regulations
 

Requirements:

Active RN license in New York State
Minimum two years Home Health experience
Strong organizational and communication skills
Basic proficiency in Microsoft Word, Outlook, and Zoom
Valid New York State ID and ability to travel
Reliable high-speed internet for telehealth assessments
